Can You Have Endometriosis With a Normal Ultrasound?
Yes. You can have endometriosis even when your ultrasound is normal. A scan is one part of an assessment, and its results need to be considered alongside your symptoms.
What can an ultrasound detect?
Ultrasound can identify endometriomas—cysts on the ovaries associated with endometriosis—and some forms of deep endometriosis. It can also help find other possible causes of symptoms.
What can it miss?
Superficial endometriosis affects the surface of tissue inside the pelvis and can be difficult to see. Small lesions and some locations may also be missed. A normal scan cannot reliably rule these out.
Does the person doing the scan matter?
Yes. The scanning approach and the healthcare professional’s training matter. An ultrasound focused on endometriosis can provide information that a routine pelvic scan may not.
What if I still have symptoms?
Arrange a follow-up with your healthcare provider if pain persists, affects everyday activities, or treatment is not helping. Depending on your symptoms, they may discuss specialist assessment, an endometriosis-focused ultrasound, MRI, or laparoscopy. Surgery is not automatically the next step, and treatment may begin without it.
Questions to ask

What types could still be present despite this result?
Would specialist imaging or a referral help?
What can we do about my symptoms now?
Your next step
Record when symptoms happen, where you feel pain, and how they affect school, work or sleep. Bring these notes and your scan report to your appointment.
